Has anyone ever done this or a similar gas to diesel conversion? What kind of paperwork do I have to do with it after it is converted to get it Safety and IMed?

    I have done two conversions: a '77 Blazer 2WD from a 305 to a 5.7L diesel, and then later to a 6.2L diesel, and then my current baby, an '81 C-20 from a 305 to a 6.2L.

It's not all that hard to do, really not much different than swapping that 250 to a 350 or something like that.

I'm not familiar with Utah laws, but here in Missouri, there was absolutely nothing I had to do legally. They don't care what is has under the hood, especially stuff made before 1981.
