Any one ever replaced a dipstick grommet and not torn it. I have replaced the one in my tranny three times. I have tried putting grease on it and anti seize. Both times it got tore up around the last 1/8 inch inside the tranny and when I am in traffic or hittin it hard for a time in the mud it starts to piss out onto the exhaust pipe statring alot of smoke. It is funny to be at a light when it happens and to look over at the people next to me and then just wave at them. :)

It sounds like you might have something stuck inside the tranny case hole, ripping the grommet as you install it. Just my $0.02.

    I had a <a target="_blank" href=http://k5.8m.com/dipstick.htm>similar problem</a> once.

Are you removing the pan when you do this? You should be able to see what the problem is if you have the pan off when you replace the grommet.

It also sounds like the dipstick or bolt-down tab may be bent and angling the end too much into the grommet when you bolt it down, then maybe hitting the firewall and making it wear out the grommet.

Are you using the right seal/tube? Both o-rings AND actual grommets were used. One won't work well on the other I'm sure. TH400 I have needed a grommet, not the o-rings the PO tried to use, unsuccessfully.
