Green Sheet for Clubs

    I've heard Grimmy speak of a "Green Sheet" that his club requires each member to keep in their glove box for medical emergencies. We have a club meeting tonight and I'd like to bring something to pass out to that effect. What info would you put on it?

    Every member of your party should be required to have one. it should at least include:
    Your Name
    Your Address
    Your Phone #
    An Emergency Contact with above info
    A Secondary Emergency Contact with above info
    Any allergies to meds
    Any chronic illnesses like diabetes or asthma
    instructions on how to use a nebulizer or insulin pin if necessary.
    health insurance information.

    that should be a good start.

    I printed one up on some lime green card stock, from what VirginiaBulldog sent. Looks good. Thanks!

    I'd tell them to make two. One for your glovebox and one to keep on themselves. Get one laminated and wear it around your neck. If for some reason you can't get to the one in the glovebox the one around the neck won't be missed. /forums/images/graemlins/thumb.gif
