I pulled this off of my '83 K20. Should fit 1983-87 (& 88 K5/Suburban/Crewcab) with the quad headlights and grill mounted turn signals. It has one small hole in the center, top of the screen and the snaps to mount it on the grill did not get saved before the truck was scrapped 
They should be available from a good hardware store though.
