Loose rotor to hub assembly

Hi guys,
I've got a '91 Blazer and while driving slowly through a parking lot with the window down I heard a clunking noise coming from the left front wheel. I got home, jacked it up, pulled the wheel and turned the hub by hand and noticed that the rotor had some play in it compared with the hub. Should I replace the rotor or do I need a rotor and hub? I've owned 3 GM vehicles with this same axle style and have never seen this problem. Any help is appreciated.
 
The rotor could be completely cracked around the perimeter and no longer attached to any part of the wheel hub. That is pretty much the only way the rotor could move seperate of the wheel hub since the wheel studs press through the rotor and into the wheel hub and then the wheel nuts would hold it all together.
