In my attempts to make the 5.3 LS in my 1959 apache look more old school, I have just recently put on a Edelbrock square bore EFI intake.

I now want to make an adapter to mount the factory throttle body to this. I have a local fab shop with a waterjet that I'm sure could make it no problem.

I am using a DBW throttle body. I would like to turn it 90 degrees so the actuator housing is pointing towards the fire wall. See pic below.

Any issues with the butterfly now being sideways?

Delmo speed makes an adapter for this exact thing but theirs has the actuator pointed towards the passenger side. It also cost $289. Their kit comes with an upper adapter to allow the use of a standard air cleaner. https://www.delmospeed.com/product/throttle-body-adapter-5-3/

So I still have to figure out what to do there

I see no reason why you couldn't mount it exactly how you have it.
But that's a lot of work for a small throttle body, you could upgrade to a 90mm LS3 for around $100. Or get a throttle body that has a square bore flange, I'm sure they're out there with GM sensors built in.

In the end its the airflow tuning thats going to be the difficult part. I would just pick what looks best to you, those TBs can be pointed any direction and in any orientation . Air dont care.

I don't see why it wouldn't work the way you're showing. A guy over on 67-72chevytrucks did pretty much the exact same thing (other than being rotated differently) if you want to see how his turned out: http://67-72chevytrucks.com/vboard/showthread.php?t=668385
That's awesome. Do I have to be a member on that site to see the pics not blurred or with the photobucket watermark on them?
EDIT: I just realized it is an issue with Photobucket

I've searched around a little bit. I did find somebody that makes a lower mounting plate for $50 but it is designed to use mustang throttle bodies (for guys LS swapping fox bodies)

So I am going to stick to making my own I think.
