poly body mount bushings

I'm installing poly body mount bushings on a 84 chevy truck. My question is do I reuse the metal bushing at the top between the body and frame mount? or do i just put the poly bushing between the body and the frame?
 
On my 76 the front 3 positions reused the factory sleeve(flat washer with a bushing that the new mount slides over) between the body and the new poly mount. The rear 2 there was new flat washers supplied to put between the body and poly mount.

Not sure on an 84.
 
Okay next question is how do I separate the original bushings? The rubber part from the metal hardware that is.
 
I stuck a screwdriver in between the metal and the old bushing and pushed it apart. BUT i have a southwest truck not much rust. You could also burn off the old bushing with a torch but it is messy and you don't wan to overheat the metal.
 
lube like pb blaster / wd-40 / liquid wrench

and work a small screw driver in and around to get loose. then just pop apart.
