One of the videos I watched in a driver safety class was about wheel/lugnut safety. It had an incident where a semi lost a tire on the right rear set of duals on the trailer and it came around the back of the trailer in a bouncing fashion over the center k-bar divider and landed where the windshield meets the roof on a Sunbird type vehicle. Killed the driver instantly. The tire wheel assembly traversed five lanes (3-1/2+ in the air) to land on this lady's car. The semi driver didn't even realize that he had lost a tire.

Doesn't have to be a semi tire. I watched a rear tire come off a sand buggy and bounce up into the stands. Caught this guy square in the chest. Fortunately for the rest of the crowd, he was a big guy and slowed the tire down enough that no one else got seriously hurt.
Funny you mention that......I watched a runaway with my own eyes today. Ford ranger doing about 50 mph (for you usa guys) lost lost driverside front tire. That thing friggin boogied down the road, finally hit a light post. No one got hurt.
