I have a Dennys heavy duty driveshaft out of my 89 k5. It has 1350 u-joints and a slip yoke to fit a 700r4 on one end. It was used with 6" of lift and worked fine for me. $100 plus shipping.

Yes it is for a 4wd. I don't see where it says 2wd anywhere. It mates up to a np241 with a slip yoke. Let me know if you want pictures or measurements.

Sorry, I see what you were talking about now, it hooks up to the np241 not the 700r4.

Is it a CV shaft or just a straight 1350 joint shaft

Just a straight shaft, no cv. I used it with six inches of lift and had no vibration from it.
