Do a search you will find all your answers that's what I did. hit the search button at the top of the page then type 14 bolt disc and the old posts about this will pop up. I'm in the process of doing mine right now just waiting for the brackets I ordered from derbinator on this board.
I just finished my rear disk brake conversion on my 14 bolt about an hour and a half ago. They work great. I have to hook up my e-brake cables though which I need to order some different length ones still. I am very pleased with the outcome of the install. I did use the 76-78 caddy eldo calipers.
