1979 k5 blazer proportioning valve help

My mechanic is having problems finding one that matches the one we need to replace. He says we need a total of 5 ports..the two I have ordered...only had 4.

Any of you gents have any ideas of where to look, part numbers... anything?

That has five ports. Two inlet and three outlet.
