That is also the work truck grill in later years. Lantzs had one but didn't pay attention last time I went.
Well, I got home and checked and some of you will probably cringe but I must have tossed it during a garage clean out. I never looked at it and thought anyone would want one (compared to the older style). Very sorry to report back negative.

Yeah sorry man. Once in a while I go on a cleaning binge and I've been living in an apartment (with about 3 single car garages) and I can not wait until I can get some property and build so I can have a set of barns like you see on Hoard..... er I mean American Pickers..........

Not sure if this got resolved or not, but what you are showing is pre-89 I think. It is paired with a different style of bezel than the later models. I have one off of an '85 burb that if you are still looking for one, I will look around and find it and send you a pictures. We can work out how to get it to you once we know if it is what you are looking for.

We're in luck. I found it. Had to venture out into the 110 degree heat, but it was where I thought it was.
Attached are some pictures of the grill that I have. Again, as I mentioned it is from an '85 burb. It looks pretty much like the one you have in the picture. It does have a couple of cracks and it looks like the attachment tab on each end is missing. The three center mounts are still attached, however. Not sure, but that must have happened before I got the truck. I don't break plastic shtuff when I remove it. By $/lb, plastic stuff is the most expensive on these old trucks. You will want to repaint it. It has some age.
So take a look at the pictures and let me know if you want me to send it to you. If you PM me, we can work out the particulars.

This is what i have. Needs a good cleaning and it looks like most if not all of the mounting tabs are there. There is 2 small pieces missing from top of grill that you can barely see
