Hey everyone, been a while since I’ve posted. Wanted you alls expertise, truck been running great but today it was running rough. I checked radiator fluid and seen that there’s a lot of black sludge, pretty thick like mud. My mechanic said I should replace radiator, radiator hoses and water pump. What would cause this issue? There’s no smoke and doesn’t over heat. Runs rough and doesn’t sound smooth, kinda shakes at idle. What’s your alls thoughts?

That sounds like a combination of very old coolant and possibly transmission fluid leaking into the radiator and mixing in.
I don't see why you need to change the pump, but hoses might be breaking down and rubber dust is mixing with the coolant.
Without me seeing it I can only guess.
Could also be oil from the engine mixing in, but usually it's coolant that has more pressure and goes in the oil
 
/\ being one issue, running rough can be any number of things. Buy any cheap gas lately?

I doubt this truck has an integrated oil cooler but if it does and there's some oil and coolant mixing, it could cause the rough running too.
The rough running could be so many things it will be hard to pin point without other corroborating information.
Is there smoke from the exhaust, is there any other signs that it's running rough?
We need more info.

No smoke, it runs rough and sounds weird, it almost sounds like a diesel truck. Oil and transmission fluid good and full

Running rough may be contaminated fuel, that’s where I was going asking about the fuel. Water in the fuel can do that

Pull some plugs and see what you have
How’s your fuel filter?
 
So I replaced spark plugs, wires, distributor cap, ignition module, and rotor and truck is still running rough. All the vacuum lines were recently replaced as well as new intake manifold and carb and pretty Mitch the whole top end is new. Truck was running great and all the sudden it started running rough. I always use premium gas. There’s no smoke coming out tail pipe. My only thought is maybe catalytic ? What do you all think?

Ok, so if we can't be there to help diagnose, can you make a video, so we can hear it and see?
Maybe we can see something you don't notice and do not post here.
 
What was the condition of each plug ? This could help give an idea of if there is a particular cylinder acting up and help narrow things down.
Did you spray carb cleaner around the base of the carb and intake - even if those gaskets are new that doesn’t mean they can’t have a leak.
Did you pull a plug wire off each individual cylinder while running to see if you could isolate a dead hole?
Next is a cylinder compression test - post the numbers both dry and wet.

I did look at the plugs and they were a brownish tint, nothing crazy looking. I will try carb cleaner today.
 
Just a little more info, when I have the truck idling the engine sounds ok but when I go to the end of the tail pipe I can tell it’s running weird, then if I sit in the truck and put it in gear the truck feels like it’s shaking and you can hear it running rough, I don’t hear any knocking or anything crazy.

That sounds like a misfire.
If you send me a video that could confirm this.
Having new wires doesn't guarantee there wasn't a defective wire or you didn't mess on up when putting them on.
Try to look at it at night, see if there's any blue sparks showing anywhere around the wires while running

Ok I’ll go out right now and start the truck while it’s still dark out. Thanks for your help
 
Ok so I sprayed carb cleaner at base of carb and it’s fine, I sprayed around manifold and there’s a leak. I’m upset because I had mechanic install new manifold and carb a few months ago and now there’s a leak. Guess I will be putting a new gasket and hopefully that fixes the issue.
