I thought the 89 k3500 was the old body style which would mean that is a passenger side drop, yes?

No, the old body style became the "R or V" in 1987. This one's out of a new body style.

Only the crew cab in the trucks continued with the old body style till 91 just like the sub and K5.
the rest of the trucks went with the new body style.
