Didn't someone recently put a 6.0L cast iron motor in a vehicle and mate it up to a hydraulic SM465? I've been trying to find information on using a 12" clutch/SM465 behind a 5.3L/6.0L and also the aluminum motors.
Yes, Advance adapters has a thicker mccloud flywheel that will make up the difference for your 6.0L to sm465.
I used the same throwoutbearing but it just barely worked. you might need an adjustable ball stud for the clutch fork.
For the slave cylinder i just fab'd a bracket to hold it in line with the stock '72 k5 clutch fork.
For a th350/400 you'll need to find an engine that had a 4l80 behind it and do some modifiying to the flexplate, get a '98 f body flexplate and modify it, or get an advance adapters or tci flexplate with the crank spacer that accepts the hub on the converter.
Another option is to forget the crank spacer and get a custom torque converter made with a longer snout or hub on the front of the converter.
