Slide hammer is how I did it, and also I believe if you do a search, someone posted another way to do it involving PVC pipe?
Make sure its the problem though, its a real pain, and on top of that, if the axle is worn down (as they usually are) in my opinion you aren't doing much good, unless you use one of the "repair bearings" or replace the axle.
