I have in the past. This really depends on the starter nose, but using shims is not a horrible thing if it gets your starter drive set to the proper engagement.
I haven't had to. I always thought that it would be ideal to swap the nose cone from the previous starter if that didn't have engagement problems. I presume the issue is normally with the nose cone since it seems like one starter won't need a shim, but the replacement will.
swapping the nose cone is an option one I have done in the past, but you must check and or replace the nose cone bushing. If worn out you might still have engagement issues.
