Did you check the blower motor resistor??...it provides the resistance to get the different speeds ,it may have roached the spring coils in the resistor....usually you will still get high speed though if one dies...try putting 12V right from the positive battery terminal to the blower motor after ensuring its grounded good and see if it'll spin then....could be a pencil or drinking straw slid down the defroster duct and jammed the fan wheel,and or blew the fuse too...
Last truck I fixed a dead blower motor on just had a blown fuse--the fan was FULL of dead mice and nest material!!...when I removed the blower motor the entire heater duct under the dash was packed solid with jute padding the mice took from under the carpet...I cleaned ot all out and flushed it with a garden hose and used some Clorox to help disinfect it..I pity whoever has to ride in that truck this winter,it still stank like dead mice pretty badly after it was all back together and working!..
