If it's on one of our TBI trucks, one of the first signs is it will shoot little pellets (catalyst media) out the exhaust. Smell could also be an indicator, but could be easily mistaken for issues like running rich etc that aren't necessarily cat-related.

But here's the easiest way to tell for sure if you cat is bad: it's stock!

I run without 'em, but we don't have emissions testing here. If you have to have them, I would definitely go with the smaller/lighter/flow-through honeycomb design.

That depends. Being in CA, you have to be careful. Check your local laws.

Will it affect truck performance? No. You can weld a straight pipe in place with no issues. I removed cats and manifolds and air pump system. Mine is just headers and mufflers. No issues.
 
The exhaust at tail pipe will be screaming hot with a plugged cat. Seen it before you could not hold your hand there very long cause it felt like it was burning the skin.
 
Rotten egg or sulfur smell is the fuel you are burning. It's not an indicator of how good or bad your cat is
