clutch fork slip off the throw out bearing?

hey guys.. just wondering if anybody has ever had your clutch fork slip off your throw out bearing... i pushed in my clutch and heard a loud clank and now the fork is no longer on the bearing :doah:could this be a tweaked fork or just bad luck?? :dunno:
 
i had it happen. either the fork was tweaked and i didnt see it or the adjustment was way out on the pushrod. things went flying and the pressure plate and bellhousing took a hit. id put a new throwout bearing clutch kit and fork in there and call it good
 
On my old Jeep, many many years ago, the pivot ball wore completely through the fork.
