Driveshaft popped off and now this...

Ok so I bought this 1987 k5 blazer off another member about 2 months ago, I haven't gotten to use it much because the coil ended up going back and it took me a little bit to figure that out. Well the truck had a rattle that could be heard from the interior but I wasn't sure what it was, sounded like it was coming from the transfer case but it was hard to tell and on inspection I didn't see anything wrong. Well after driving it for a few minutes above is what happened. Below I listed all the mods that the previous owner had done and I just not sure why this happened, is the angle wrong, did a bolt come loose, any suggestions would be great as I want to fix it and make it so that it doesn't happen again.

bi 350, runs good
headers
full dual 3" flowmaster exhaust with 40 series
sm465 4 speed granny gear
np205 case
$1500 worth of HAD driveshafts and yokes

suspension:
6" front springs
stock rears w/ add a leaf
diy shackle flip
6"/7" shackles
custom ladder bars

axle:
(1 tons with 26k on them)
d60 front

tires/wheels:
44/19.5 tsl boggers (brand new)
15x14 chrome pro comp wheels (brand new)
 
looks like the drive flange came off.did the nut strip off and alow the flange to come off.or just come off because not proply torqued to hold it on.
 
i bet the nut is traped in the flange and shaft . i would bet the nut backed off and was not tourqed to spec .

when you do this much major surgery you need to recheck stuff on the first big test drive and then a few drives after.

i would now fix this and call jesse at had and then have someone other than your self go over this rig with a fine tooth comb and look for the problems and then go from there.

before this happened were you feeling a knocking noise like under your the center of the floor?
 
Thanks for the advice guys, well before it stopped running we heard a rattle noise that was quite loud directly below the shift/transfer case area, we looked and I didn't see anything loose so I figured I would check it when I got a chance to put it up on our lift and drive it with the wheels off to see if I could locate the noise but then the truck broke down and sat while we were figuring out the electrical gremlin. Once we did my brother took it up to go to a show and I forgot to tell him that we need to find out what that rattle was and he didn't even make it 5 mins down the road.
 
i might have a idea on how the flange or nut fell off.

today i was cleaning up my 205 tcase parts for full reassembly. i have 2 flat flanges for my build. he ships them with new flange crimp nuts. thay have there own washer built in to the nut.

upon inspecting my stuff after seeing your problem i see if someone would reuse the old washer and the new nut supplyed and then assume its good to go you will have a big problem.

the washer if reused will not let the nut get in to the crimped section to hold on to the output shaft and stay tight. you can only use the new nut on the flange from had and not the old washer also.

hope this help you find out what happened. and did you remove the flange from the shaft yet and see whats left in the center hole and see how it failed?
 
yea that would make sense, yea we pulled it off today and will have to order a new flange as it got ruined when it came out and banged around but thankfully thats all that seems to be wrong so monday will order up a new one for 100 bucks and install this one properly and we should be good to go, thanks so much for your help...
