I saw an extended cab in the 73-87 body style yesterday. i know i'd seen them in the past, but its been a long time. how many of them were made? was it a factory option or a custom number.

They are strictly custom built, 2nd gen ext cab's were never made by GM.

Like nvrenuf said, none were made by GM, all are custom made.
There was an article on one in Fourwheeler a few years back.
I am working on something that GM never made either.
SOme people like to have something different and the only way is to make it.
