Wanted Heater only control head

I’m looking for a cosmetically nice heater control assembly. No cables or wiring needed, I just want something that looks good to fill the hole.

Not sure if the fonts are different over the years, this is for my 91 Blazer.

How accurate does it have to be?
I am thinking the flat shape medium flat rate box? It's 11.5×13 and I can stick the heater control in.

It can actually be pretty small. What I truly need it about 6" long piece with a complete raised rib, maybe some of each valley so I can trim to size. I screwed up with a holesaw and need to fix the hole.

Yeah I have the bed floor all there one of the bottom cross members was rotted but the floor is clean.
I will start from the edge and cut pieces for those who need
