Help Confirming Front Axle Leaf Spring and Shackle Bushings

I have a set of Tuff Country EZ Ride 4 inch lift leaf springs on my truck, as well as an Off Road Design shackle kit on the front axle (D60). I need new bushings at all 6 locations (front eyes, rear eyes, frame side shackle). Is anyone able to give me the needed part numbers and suggestions on where to purchase, as well as if poly is even the best option these days? Based on work I did this weekend, I believe the large front eye needs an M02041, frame side is M02136 but the rear eye is unrecognizable.

I have not had much luck, as seen below.

Some Background on my efforts to date:
I originally thought it was just the leaf spring bushings needed and reached out to Tuff Country for their recommended bushing replacements. They would not sell me the bushings directly but told me to source a Daystar KG02315BK kit.

I received a complete set of poly bushings with sleeves, all matching, with a part number M02157 embossed into the bushing itself. The only difference was the sleeves, two were thicker walled with the same OD relative to the other two.

I set to work and removed the front eye bushing first, only to realize it was way bigger than the bushings I received. I checked the local auto parts places open on a Saturday and nobody had the correct bushings (or even carried any Daystar poly bushings). I re-assembled and started on the rear, as those looked to be correct. Once apart, it was apparent that the flange thickness was increased on the new bushings but given how worn the originals were I didn't think much of it. After sitting in the freezer, the new ones slid right into the rear eye and the frame. However, when I bolted the shackles together it become apparent the bolt was going to be too short; should have been my first clue. After getting longer grade 8 bolts I discovered that the TH350 linkage was going to hit, so even if I wanted to keep using these bushings I really can't. Given the different part number, the flange thickness difference seen must be not completely from wear but actual use of a thinner flange to allow for this clearance.

So, back to trying to source the parts. Searching the part numbers I could discern, I found a spring shop in Arizona that stocks the poly bushings and sells them separate from the complete Daystar kit. However, I would need still need confirmation of the rear eye bushing part number first as I it doesn't look like I should keep using the ones originally recommended. I did try searching the forum and the internet for the part numbers I had in hopes that I could find a reference to the missing part number, but so far it's mostly just found old posts with limited info.

Last I knew, the ORD kevlar bushings were recommended. If you measure your old bushings, they can be sure to match it up. If you are still running stock-size upper bushings, now would be a good time to swap to the larger size using the ORD FUSH or the one from DIY4x (although I'm not sure they offer that kit anymore)
 
Thanks for the quick feedback. Sounds like a call to ORD is in order... half their stuff is already on the truck. I probably could just order the parts straight from the website based on their detailed descriptions but it would be nice to confirm the part numbers first.

I looked at the FUSH kit when I was trying to see if they called out part numbers for the actual bushings on their website. Given how long the first set seemed to have lasted and my current usage (relatively minor wheeling, mostly on-road until I move in a couple years) I think I'll stick with the stock uppers for now.
 
Be sure to tighten the bushing down tight to the sleeve. I 3rd the ORD kevlar. Have them complete on my truck, even swapped the fronts from 1 set of springs to a new set.

Sorry I did not snap a pic of the stock ID. I don't know if I still have the old weldments.

I am pretty sure the stock ID is published on ORD site.
 
Project complete. Order placed on Monday, parts came midday Saturday, had the truck back together in time for dinner Saturday night.

My 40+ year old body is not so happy with me still today, but the work was worth it. Drives like a different truck, steering is much tighter and consistent, with no death wobble yet!
