Help! Engine has a rough idle and has a shake

At about 1987, all of the blocks were cast for roller cams, but the trucks still ran flat tappet for about another decade. If your engine is 87+, you can convert to an OEM type roller. If it's older you need the retrofit type kit, which is more expensive. It's not uncommon for engines to get changed in vehicles this old.
 
Any manifold vacuum source. The front of most carbs have a manifold vacuum port.
 
update - asking shop to just instal a new Comp cam cl12-206-2. I will share update when finished..
 
Edlebrock Performer will prob be your best bet. Be sure to get the egr one. It will except your carb.

with the cam and intake you should see a difference. Depending a lot on how well your exhaust flows. Hopefully the computer will learn and adjust the ccc carb mixture to take advantage.
 
So was there evidence of a flat lobe? How did the oil look?
 
I take it that is a Q Jet CCC? Make sure the electronic readings (dwell on carb electronics) are in spec. On the HEI cap and rotor, did you check for carbon streaking? As you are in CA, be careful of the cam you choose and make sure it will meet CA smog standards. I'd use a vacuum gauge and see what the readings are. Sounds to me that you have a bad intake valve either not adjusted correctly and/or a lobe going bad. Best way to check that is pull the valve covers and run the engine and look at the valve lift on the rockers and see if they are equal. (NOTE: be careful of oil spray when you do this). My guess is a bad intake valve/lifter/cam/rocker. Rotsa Ruck! BTW...Roller cam is the way to go.
 
What roller cam? Do I need to change block?

Yes you can add a roller cam to an older block, you just need the retro fit tie bar lifters. You also need to use a cam button and set your camshaft endplay.

I see this now but it sounds like you are getting it figured out, let me know if you need more help.
 
Wanted to share an update. I had the entire top end rebuilt. new cam, heads, exhaust manifolds, etc. I changed connectors, new vacuum lines and rebuilt carb. runs good. I put in a less aggressive RV Cam and wish I went more aggressive. At this point, I am leaning towards a new GM eCrate performance motor. thinking it will feel like a new modern truck, and I can trust it on long drives.
