I am gonna go to 4wp to go get some shocks and i plan on gettin 9012's and the extened mounts for them to fit.Anybody running them and how much did they pay for them ?

Running them, paid $60 each direct from the local store of 4WPW with our local Oregon Bushhackers discount.

I read on this board last week that someone got them for $58.95, not sure if it was internet, included shipping or what, but remember seeing it.

Nope, didn't go that route. Those are Ford F-250 parts, don't think they are available from 4WPW. The rear ones are 9012's (as recommended due to the eye to eye measurement), fronts are 9118 (I think). Don't have the extended mounts, might never have them. If I do in the future I'll sell the 9118's from the front and order the 9012's. Price was the same regardless of model number 9000.
