Idle Problem

Having trouble figuring out this idle issue.
The truck idles good when cold (higher rpm) and does fine in park when warm. Once in gear it stalls almost every time. Seems like reverse is worse than drive.
The IAC is now at least 8 even when idle is low so I don't think there's a vacuum leak.
I've changed: IAC valve, TB gasket, intake manifold gaskets, a couple of vacuum lines, o2 sensor, spark plugs, cap/rotor

What are the idle settings supposed to be? it seems like the iac counts don't go up even when idle drops to 500.

Any other ideas?
 
I unplugged the EGR valve and it doesn't seem like the solenoid is leaking.
I connected engine vacuum to the valve and the idle gets noticeably worse. I guess the EGR may still be leaking slightly... I'll try to take it off and replace it with a block off plate.

Searching around here some people have suggested TCC not releasing. But wouldn't this prevent me from even keeping it running with extra throttle applied? Or is there quite a bit of slip in the TCC normally?
