Is my front diff yoke messed up?

Ok, I just replaced the front u-joint on my front driveshaft about a month ago, and when I was putting the shaft back in the truck I found that the driveshaft yoke on the front diff only had a locating tab on one side to hold the u-joint cap in place, the other side was just smooth, I didn't have time to worry about it then so I just bolted the clamps on and it seemed to be holding the cap in place.

I was using 4 wheel drive this last weekend and it was smooth at first and then after a while I started getting a bad vibration above 15-20 mph, I took a look under the truck when I got home and I found that the u-joint cap on the smooth side of the yoke had moved out a little bit and I could wiggle the shaft about 1/8th inch or so.
do I need to replace this yoke to fix this or am I missing something?
 
If my memory serves me correctly I had the tab on both sides of my 10 bolt. Which axle do you have?

89 K5 Silverado, 4", 33's, 3/4 ton axles

Well the caps have to be held in by something. If there is no snap ring, then there has to be a tab. All of the yokes I have ever seen have had a tab on both sides. Maybe a former owner threw a driveshaft and just did a quick fix?

that's kind of what I was thinking, that they just did a quick fix to get by before, I have a feeling the missing tab is why the last u-joint blew out on me. Also, it is the stock 10 bolt front axle.
