is my tranny gone?

when i am accelerating the tranny makes a horrible vibrating noise and jumps around..
is she a gonner?

i stops when i shift down into 2 or 1.. thats how i have been driving it for a while.. everytime i start and stop i have to shift up and down..but now its starting to do it in the lower gears..
