loud clank

Lay under the truck and rotate the driveshaft by hand. Listen/look for the clank location.

might be the u joint. I'd check that one out.

Throw it in neutral and also rotate the dirveshaft at the back of the tcase, check for excessive play. Not sure what kind of truck it is though. Sounds like a ujoint.

1976 k5 350/350/203 14bff. it almost sounds like it might be between the u joint and or the gears.

Might be safe to just change out yer ujoints, Ive had them be completely tight, no play but make noise and be totally screwed. Check yer pinion nut, I have seen those come loose and make make noise

I have a clunk too, but have replaced all u-joints, I think its in my t-case, possibly the chain.
