Man,I'm glad I didn't buy one of these....

Looks like the bolts didnt have enough thread engament and it just pulled out the bolts. Then the torque bent the ears of the adapter.
 
Looks like a combination of the above two posts contributed to the failure. I really don't see much for thread engagement on those bolts. It appears as though the only part that stripped out was the very tip of a couple of them, so maybe they were backed out in the first place. :confused:

You're quite correct, the paticular situation with the customer was too short of bolts used and lack of lockwasher/loctite, the parts were replaced without question and overnighted.

A valid question - but the 4 bolts are enough to retain the adaptor kits together and have worked well for the 95 other adaptor kits in use today.

This is another benifit of the multi plate design. NWF just need to send out the plate that broke, not the whole adapter. Maked life easier.

I really wish I would have held off for one now (and the bling alloy D60 arm).

It still appears that you only get about 3/8" to max 1/2" thread engagement into the 205 plate. With the Correct length bolts how much thread engagment do you get?
With only four bolts producing the clamping force this seems a little lite IMO.

Way to go with the Customer service.
