I just noticed last week that I had a manifold bolt missing on rear driver side port. So I installed a new bolt and now the other bolt is missing a day later. WTF?

No big shock there. Ive got heddman headers and have tried various header bolts and none will stay tight more than a month. So, once a month I go and retorque them. Wouldnt be bad except the ones by the firewall sux. Not enuff room for a socket and can barely get a wrench. Ive tried thru the top, thru the bottom, etc etc. Never had this issue with Cast manifolds.

I've never touched my original cast manifolds and now suddenly I have bolts falling out...

As for that socket problem .....Ratchet wrenches.....Works awesome for me!!!

on my headers the bolts are so close to the tubes that a ratchet wrench is too thick to get on it. so far the only thing that I have found that I can get on the bolts is the open end of a wrench. for the bolts by the firewall I have a special wrench that I heated with a torch and bent until I could easily tighten the bolts.
