Please identify this locker and a question

I bought this locker the other day. Can anyone identify the brand?









I also have a Limited Slip Dana 60. I thought all I needed was an open carrier but read this on the installation instructions for a Lock Right it says the following. If so, am I screwed?

The LOCK-RIGHT is designed to fit into standard open differential cases only, not into limited-slip cases. If your vehicle contains a limited-slip unit you will need to purchase a standard open differential case, side gears, thrust washers and long pinion shaft (and also the two short shafts if it is a four-pinion unit) before proceeding.

That is correct. The lock right replaces the spider gears so you will need an open carrier.

I got an open carrier with the locker but I was confused when the instructions say I need an open CASE, open gears and long pinion shaft.

Here's what came with the locker. I have measured it and it is a correct 4.56 and up carrier.
