just the bolt-in? my bet is cracked frame...
When you turn, especially with crossover the springs flex side to side a small amount. If your sliders are worn or thing or theres a large pit of bubble of rust leafs can make clunking noises when shifting one each other.
Get under her and have someone saw the wheel back and forth.

Not sure what you mean by sliders. The clip that holds the spring pack together?

Picture? I've got no hard plastic pieces that I know of.No the hard plastic pieces at the end of each leaf spring that help them glide over each other. They go bad, get worn thin, make clunks.
if you didn't actually pull the box off the frame, i wouldn't rule out the frame.. they general stay under, and around, the box feet for a long time before they rip the frame in 1/2...
I have one that is barely crooked. When the girlfriend gets home I'll have to check if it moves when the wheel is turned.Doesn't hold it together, per se, that's the center pin's job. It/they just keep the leaves from rotating. (Well, they might also keep the pack from over extending ... maybe...)

Had the same thing on the suburban. It may be the bushings are worn out on your sway bar (if you still have yours). I replaced mine with energy suspension greaseables no more problems.
Never even thought of that myself, but the ex brother in law was a race tech for nissan and diagnosed the problem. I guess when you turn the wheel moving or not it still preloads the bar a bit.
