I am using a Griffin direct fit LS 4 core radiator to do 6.0 swap. Both inlet and outlet are on the passenger side.
Upper water pump neck size is 1.325" with the radiator upper being 1.262". Lower water pump 1.58" with the radiator 1.52".
Typical upper hose for the 2005 2500HD 6.0 donor is a hose that crosses over the top of the radiator so that won't work.
What have you guys used for your swaps when using a non factory old school cross flow?

I am going to list what I find as I mess around and research.

Upper Gates 21439

Close one - I didn't have a local auto parts that carried Gates, so I ended up with a Dayco C71588 for the upper and a D70649 for the lower - http://67-72chevytrucks.com/vboard/showthread.php?t=601424&page=3

I've also got a Griffin radiator. Not overly happy with it but that's another topic. (EI, fans are too loud and upper port is too high, should have been made in the side of the tank instead of the very top)

Anyway. I've got

Lower: 60374
Upper: 22826

I think they are Gates or Goodyear, can't remember. The lower is for some Ford truck. I can't remember if I had to trim or not, but it's always an option. This is on a '68 C10 but I think the radiator is the same.
