How does this look? How far up should the patch go on the panel? Inner structure looks good.

I would make sure you have the patch panel in-hand before cutting too far. Ideally you don't cut out more than the patch will cover (although sometimes unavoidable if there's rust past that area).

Given that, I usually cut out enough to ensure only clean, full thickness sheet metal remains. It also helps to give yourself room to clean up any surface rust inside the area (such as on that inner splash guard piece in this case). I like to get stuff like that as cleaned up as possible and hit with a coat of spray paint before I close it back up to prevent future rust. It's also typically easier to cut out normal-ish shapes as much as possible. Cutting the patch to fit is easier if it's a rectangle than an oddly shaped blob.

In this instance above the wheel opening specifically, it looks like you may not yet have cut off the bottom lip of the bedside, which bends underneath that inner splash guard? There should be spot welds between the bedside lip and the splash guard, which you can see if you stick your head into the wheel opening and look straight up. Hopefully I'm explaining this well...

I'd recommend getting that section of the bedside lip off by cutting it at the sides of your patch area and drilling out the spot welds I described above. The patch panel for that area should come with the lip, so it's easy to replace.
