Which is better the late model geared or the old style ? Will 1 red top optima start my truck consistently ?

The gear reduction style is better.
1 battery might start it but not consistently.
 
I run a pair of AC Delco 1000 CCA batteries and the 28MT. I've also noticed the 28MT's have come down in price alot. I paid well over $300 for a Bosch Remanned 28MT with a 5 year warranty.

I also did the heavy gauge battery cables with soldered connections.

FWIW, i run 2 exide 950 cca batteries (same thing as a red top) and have heavy guage wires soldered and whatnot, and the stock starter - it starts up amazingling fast. faster than any gas engine i've ever started.
 
In your climate, 1 battery will turn that starter I sold you (old style) for a long time. I used only one battery in my truck in 10 degree weather with no problems using that starter. Just run 2 ga cables to a top post battery and keep the glow plugs in good shape. I melted a few side post terminals off the battery :doah:
 
I bought a 28mt and a new exide orbital / optima knock off same price deep cycle battery , works great and now my truck even runs !:D
