Do the ports on the steering assist cylinder have to be pointing straight up?
If they do, is this for bleeding purposes only?

I have the ports pointing to the rear and I'm wondering if the cylinder is unable to bleed all the air out this way.

I have followed the bleeding procedure but am unable to get the cylinder to work.

I have the PSC steering box, cylinder and pump.

People typically point them up but, like you, I think it's for bleeding. If you unbolted the cylinder and bled it, it should be ok to mount it however you want afterwards.

Fwiw, hose routing matters so that you don't have the ram going one way while the box is going the other.
 
I don’t think it matters really. If the ram sits low, it’s to protect the lines. If the ram is in a high steer location, the lines are probably out of the side for clearance issues.

I will echo making sure the lines are correct. I bled things with the tires off the ground and engine off. Rotate steering back and forth a million Xs and the reservoir level should fluctuate from right to left when things are working right.
 
if you plumb the assist backwards it will blow fluid out the reservoir, I know this first hand so that's not likely the problem.
