Strange 4.3 noise...

I'm working on a 98 S10 pickup, had a spun bearing. I flipped the motor over and did a basic bottom end rebuild. Crank, bearings, oil pump. I've done probably half a dozen like this, and the lifters always make noise at first. I'm guessing all the leftover crap from worn out bearings clogs them up. A can of Valve Medic has always fixed them up in a matter of minutes.

This one still makes noise, and I've run it a bunch. It sounds exactly like a lifter, but here is where it gets weird... It really only does it under load. In neutral you can rev it to the moon and not hear it. Put it in gear and go, and there it is. Coast at higher rpm and it's gone. I've already replaced the exhaust manifold gaskets, sometimes that can have a metallic sound. I made sure the flex plate was in good shape while the motor was out.

It doesn't make sense. The lifters don't care if there is load on the engine. They don't even know if it is under accel or decel. The sound is coming from what seems like the center of the engine. I'm gonna go out there and replace them, but I'm not super confident that this is the problem.

Any thoughts?
 
Loose torque converter bolts maybe ?..
Is this a V6 with a balance shaft ?..
 
I just had the motor out, so unlikely on the TC bolts. I've had that problem before and it quieted down under load, made noise at idle. This is the opposite.

Yes it is a balance shaft motor.
 
Exhaust leak? manifold crack you haven't noticed yet? ticking under load is a classic exhaust leak description. I had an 88 GMC with a TBI 350 back in the day. The dealer did a rear main seal under warranty, but it came back to me with the same 'tick' you're describing. Turned out the tech broke one exhaust stud and that was all it took...
 
The last super cheap engine rebuild I did only tapped in gear under a load. Ended up blowing 1 @ 2 rods out of the pan. Being in gear puts a different load on it vs revving it up in N.
