TBI 350 engine surging problem

My 89 K5 sometimes when I first start it will surge, it will surge so bad it will quit. I can shut off the engine and restart and it runs fine. Anyone had this issue before that could shed some light on the problem would be appreciated.

I have a similar issue but I have headers and a non-heated O2 sensor which I believe is to blame but don't know for sure.

I really don't think the headers are your problem, I dunno, just from personal experience, I have long tube headers on mine, back when it ran, it started and ran fine even with the o2 sensor unhooked.

Drove it to Indiana and had a different issue at times, accidentally pulled on the o2 sensor wire, unhooking it. I continued to drive it, yes the CEL was on, but nothing ill about the way it ran, maybe used a bit more gas, that's about it.
 
Possible Idle air control valve. I had a TBI surge on startup die, restart and that was the culprit. Does the CEL come on when the surge occurs? I cant remember off the top of my head what the values are but you can check the electrical signal at the Idle air valve and see if its responding correctly.
