throw out bearing

I need recomendations for a quality throw out bearing. I replaced my clutch and everything related to the clutch back in May and now my TO bearing is bad. I have a Centerforce Dual friction clutch with a centerforce TO bearing to match. What do you recomend as a high quality replacement?

BTW this is a manual clutch.
 
If it went bad that fast, you may have not had enough free/end play. The bearing shouldn't be touching the pressure plate when you're foots not on the pedal. 0.060" of gap translates to at ~1/2" of movement at the end of the fork.

In any case, doesn't CF recommend only using their bearing for the dual friction clutch?

Yes CF does recomend to use their TOB on their dual friction clutched

I've kept the clutch adjust propperly with about 1/2" to 1" of free play.

What I think the problem is though is that I never put the dust shield or fork boot back on after I replaced the clutch. With a lot of off roading there was surely a lot of dirt and water to get in there. Also I failed to notice that the bellhousing bolts loosened up to the point were the flywheel was hitting the bellhousing and the clutch fork popped off of the piviot ball and caused soem erratic clutch functioning that led me to finally find the loose bolts.
 
Ah, well it sounds like you know what to do. Some of the release bearings I've tried had plastic retainers on them and I didn't like those very much. I broke one installing it. I found one that was all metal and like it much more.

the centerforce one is all metal, and coming from centerforce I assume its decent quality. I guess I should have kept a closer eye on those bolts.
