Truck doesn't like running when cold

Just like topic says every morning when I start my truck it runs like crap, backfires and I can smell that its running rich. At lunch today it also took some warming up before it ran normally since it was colder than usual.
Another problem to add is that the idle is pretty high usually between 1000-1500 rpms. If I'm in drive or reverse it will idle at 1000 ish. If I go into neutral it will jump to about 1500 ish.
On my way to class just now it was idling high so at a stop sign I turned the truck off and on and it idled fine. This was after driving for about 15 minutes.
Anyone have any idea where to start? In class right now and using my phone so hopefully it isn't hard to read.
 
I'll start by guessing the IAC is beginning to act up.

A bad EGR can cause problems like this too.

Could also be a combo of the two or even a whole bunch more issues but I would start there.

I had a bad EGR one year, replaced it and noticed that idle went to normal, :doah: I always knew the idle was higher than it was supposed to be, but never bothered with it, cause it ran fine :laugh:

Now i get a slight surge when cold, like a bigger cam type surge if i go to drive before it runs for a few minutes.
