Because if it's a CA truck, then it will have to meet more strict requirements, but if it's a federal meaning non CA then it's easier.
And we bitch about smog because there is no expiration anymore, a truck could be a hundred years old and you'd still be required to test.
The last year that was exempt was 75, so 76 and newer will alway shave to be smoged.
But the worst thing that we complain about is the fact that your vehicle could pass the sniffer but it still has to pass a visual where the parts that you have must be CA aproved and certified with a compliance number, or you will be failed.
So it's not about running clean, but about doing it their way.

In CA a car is required to meet the emissions that came on the vehicle. Also any mods as stated by Ice, must be CARB stamped.

If your motor is newer than the vehicle, then you must first have all the smog of the newer motor.

Its a PITA, but yah it can be done. Just depends on what was stock on the truck.
