Lifted? How high can you go?

Yes - all square 30 series ( that I know of ) came with rear downward u-bolts wether C, K, V or R and SRW or DRW ( D70 or 14-B ) including Cab Chassis’s - I think it was to utilize common mounting hardware across the board for the option of the rear sway bar retention extra pieces/plates on the 2WD 1-TON’s and the top retaining bar that worked on standard leaf packs as well as those with auxiliary overload leaves.
 
My 1991 V3500 crew cab has roughly a 10" lift with 38" tires. It has a 1" body lift from ORD. The front has 8" Sky Jackers with a set of zero rates from ORD, and a set of home made 1/2" zero rates as well. The rear has a DIY4X shackle flip with a set of 4" blocks. It rides a little stiff with only me in it, but if I put 300 to 400 pounds worth of people, or cargo in it then it starts to ride OK. Definitely need to have cross over steering with this much lift.

Easier to go that high in a crew cab.
The wheelbase helps get drivelines happy and it a lot more stable.

My crew on 5-6" and 40"s drives no different than my stock 2500hd at 85 mph.
My old K5 with 6" or maybe a couple inches more (looked taller), on 36"s was almost scary at only 70. Lol.
(Too be fair the K5 was a POS and the crew has had everything redone in the steering system.)
