Fires right up, but have to stay on throttle to keep running. Usually about 2-minutes, then it will hold idle without stalling. Runs rough with backfiring during this time.

350 w/tbi and 700r4

Has new fuel pump, fuel filter, coil, thermostat sensor.

Code 45 might be an O2 sensor issue:

That may or may not actually be the issue, but an O2 sensor is like under $20 last time I got one. Might be a place to start, but usually that's not used until it's warmed up a bit.

If I had more time I'd look into it more, but unfortunately I've got a packed day.

Any way to test fuel pressure regulator?

Mine was the TPS when I had that same issue. Would run rich until heat started being generated.
