It's a 5 speed then. NV4500 with the Granny Low, 3 regular forward gears, and OD. Reverse obviously.

Good thing it's not a 1500 or 2500LD...they got the crappy Getrag units at about that time.

clean truck man. procrap is right and those tires are SSRs btw. i also HIGHLY doubt it has a 208... rather it should have a np241 or if your lucky a BW4401 (idk if the BWs where exclusively 3500 model units or not)

Should be a 241. There never was a driver's side output 208 under Chevy's and the B&W was only under 1 tons.
