Ok I can't with hold these pictures from you guys any longer.
Keep in mind these pictures weren't taken with a very good camera and lighting wasn't the best and its been raining the past several days so it is a little dirty from road debris so the shine isn't at its full potential.

Ya the box sits on a full 3" body lift blocks on only 4 out of the 8 mounts while the cab sits on about 2 1/3 inch body lift because any more and the clutch linkage would get messed up. So the bed needs to be lowered down a bit. The frame may have some sag in it but what you see there is because of cockeyed mounts.
I plan on addressing the mounting issue later... which means it might never happen.
I'll get a side picture tomorrow so you can see it better. Along with pictures of the mounts.

It was Mechanic Street Auto Body in Bellingham, MA. They are a small place but my neighbor is good friends with the head body guy there so I got my truck in there with a higher priority.
Also they didn't just shoot some paint on it, they completely dissassebled the front clip, they took the bed off, and they did a crap load of work smoothing out all the seems and aligning the body panels. All the very tedious stuff no one wants to do. They did a good job with it.

I got some more pictures today. Also I realized those other pictures weren't taken on a level ground so that may make the truck look more like the frame sags more than it does. Also looking closely to how the bed mounts the rear passenger side one sits on the frame that is bent up slightly so it make the passenger side look even worse and far as bed to cab alignment.
So here it is on level ground from the drive side:
Here is the bed mount:
Here is the body mounts as they are slathered in grease to stop the insane squeaking coming from them. Basically they are new polly mounts with a 1/4in spacer on top then a hockey puck. (When I put the body mounts in like this I was very fustrated and pissed off)
These are all the hockey pucks I took out the truck. Before I started messing with the mounts they were all just hockey pucks. Total of 28 pucks and about 26 are shown here:
