Rockers moved and chewed up valve

I have Roller Tip rockers on my 350 and went to adjust them last night, noticed that 2 of them had moved sideways and were not sitting on the valve stem correctly. One chewed the valve pretty good and discolored the rocker(I assume from heat). What would cause this to happen? I am going to pull the head and have it fixed but am wondering if I need to get some different rockers. I have screw in studs, 1.5 rockers and I have a performer cam.
What do you think???

what heads you running?? I know some heads such as vortecs and some 86 up heads need self aligning rockers. Im not sure if they would fix it, im just thinking out loud mostly.
