Where can I get one?
Out of a conversion van. There are a few companies that do them, can't think of any off the top of my head right now though. Look in junkyards, I'm sure you could find a few.

You can get aftermarket seats like that from many vendors - just Google "pickup truck seats". They are probably not really "K5" seats in regards to rear seat access. It might work better to fit the K5 brackets to the aftermarket seats for this reason, but would add complexity to the install of course.
