Custom. At one point DIY4X offered tank fab services.....
 
Ran mine with the flanges below the frame, I wanted to be able to drop it if I needed to. I lose some clearance, but I an willing to lose that for serviceability. Build a beefy skid plate and be done with it.
Of course my truck most likely won't see the same type of use yours will.

I wasn't losing any serviceability, in fact it would be easier with the pump available under an inspection cover like I was planning. I wanted the ability for the whole tank to be removed through the bed if needed (think the opposite of a k5 with straps on top of the tank).

However I spent some time playing with the tank in the old location where I had it and it makes more sense lower and I'll retain the bed. The skid was going to happen either way, only difference is that I'll need to make the skid and framework removable. You can see in these pics how much lower the leafs and shackles hang down so I think I'll be fine. Plus I can still haul 4x8 sheets of plywood #crawlandhaul
